How to Apply for a Medical Marijuana Card

Step 1
Schedule a Medical Consultation

Schedule an initial consultation with a registered medical cannabis practitioner. Find a practitioner on the Kentucky Medical Cannabis Program Website.

Step 2
Attend your Consultation

Go to your scheduled appointment with the practitioner. During the consultation, the practitioner will evaluate your condition and issue a written certification if you qualify for a medical cannabis card.

Step 3
Apply for your Medical Cannabis Card

Apply for your Medical Cannabis Card on the Kentucky Medical Cannabis Program Website. Register an account and upload the completed Patient Application Signature Page and pay the $25 application fee.

Medical Marijuana Card FAQ

To qualify as a medical marijuana patient in Kentucky, you must meet these requirements:

  • Residence and Age: Be a Kentucky resident and at least 18 years old. (Patients under 18 can qualify with a parent/guardian as their caregiver.)
  • Medical Condition: Have a qualifying medical condition certified by a registered practitioner.
  • Medical Certification: Obtain a written certification from an authorized medical provider (physician or APRN) stating that you have the qualifying condition and could benefit from medical cannabis.
  • No Disqualifying Felony: Not have any disqualifying felony convictions on your record.

If you meet these criteria, you can apply for a Kentucky medical cannabis ID card.

Kentucky’s law lists specific qualifying medical conditions for which medical cannabis may be recommended:

  • Any type of cancer
  • Chronic, severe, intractable, or debilitating pain
  • Epilepsy or any other intractable seizure disorder
  • Multiple sclerosis, muscle spasms, or spasticity
  • Chronic nausea or cyclical vomiting syndrome resistant to other medical treatments
  • Post-Traumatic Stress Disorder (PTSD)
  • Any other condition approved by the Kentucky Center for Cannabis

If you have one of these conditions, you may be eligible for a medical cannabis card with your written certification.

In Kentucky, the application fee for a new medical marijuana card and renewal is $25. 

  1. Visit an authorized medical provider. Consult with a physician or advanced practice registered nurse (APRN) who is authorized to recommend medical cannabis. If appropriate, they will issue a written certification for medical cannabis.
  2. Complete the state application. Submit an application for a registry identification card (medical marijuana ID) through the Kentucky Medical Cannabis Program’s website. The application requires personal information, a written certification from a registered practioner, and a $25 fee.
  3. Receive your card. Once approved, the state will issue your Kentucky medical cannabis ID card, valid for one year.

Yes, but with restrictions. Patients under 18 cannot directly obtain a card, but they can qualify through a parent/guardian acting as their caregiver. The minor must still have a qualifying condition and a registered practioner’s certification. The parent/guardian must consent in writing and register as the designated caregiver, responsible for purchasing and administering the medical cannabis. Minors are not allowed to enter dispensaries or purchase it directly.

Kentucky allows visiting patients to apply for a temporary medical cannabis ID card or provide the dispensary with an out-of-state medical marijuana card and evidence they have been diagnosed with a qualifying medical condition. Once approved, you can purchase up to a 10-day supply of medical cannabis at a time.

No. Home cultivation is not allowed for patients in Kentucky. All medical cannabis must be purchased from a licensed dispensary.
