The LiBBY Study
The LiBBY study (Life’s End Benefits of Cannabidiol and Tetrahydrocannabinol) tested a combination of CBD and THC in hospice-eligible patients with Alzheimer’s disease or other dementias who were experiencing significant agitation. Kentucky served as a trial site, with involvement from Dr. Greg Jicha of the University of Kentucky’s Sanders-Brown Center on Aging.
In this phase 2, randomized, double-blind, placebo-controlled trial, participants received a carefully dosed oral oil containing THC and CBD or a placebo for 12 weeks. Many visits took place in patients’ homes to accommodate those who were frail or bedbound. Results showed clear reductions in agitation measured by the Cohen-Mansfield Agitation Inventory. Improvements appeared within about two weeks and held through the full study period. Clinician ratings indicated that a large majority of people on the active combination improved, compared with a much smaller share on placebo. Patients were described as more awake, alert, and comfortable, with corresponding benefits noted for caregivers.
Dr. Jicha called the reduction in agitation scores “striking” and emphasized that options for preserving dignity and quality of life in the later stages of dementia remain limited. These findings add to evidence that cannabinoids may help ease certain neuropsychiatric symptoms of dementia, particularly agitation, though more research is needed.
For Kentucky families, the results come with an important practical note. The state’s medical cannabis program lists specific qualifying conditions, including various forms of cancer, severe or chronic pain, epilepsy and intractable seizures, multiple sclerosis, muscle spasms, chronic nausea, post-traumatic stress disorder, and additional conditions clarified in 2026. Dementia itself is not currently included. Dr. Jicha has described this as an oversight given how common dementia-related distress is among hospice patients.
Access still requires certification by an authorized practitioner based on the conditions defined in state law. Some individuals with advanced illness may already qualify under categories such as terminal illness or severe chronic pain, depending on their full medical picture. The exact research formulation differs from products available at dispensaries, and individual responses vary.
